Which AA attendance sheet should I download?
All four are free, print on one page, and can be downloaded blank as a PDF or filled with the meetings you log above. Blank sheets come ruled with empty rows so you can sign them in at the meeting. Only the court card asks for a case number and a signature.

An AA sign-in sheet and an AA attendance sheet are the same thing: a page listing the date, group name and topic of each meeting, with somewhere for the chairperson or secretary to sign. A court card is that sheet made formal enough to hand to a probation officer. If you are not sure which you need, download the blank court card, because it carries every column the others do.
